BF mummies and codeine

What do you make of this?
I'm currently on prescribed paracetamol and codeine, and would have continued to take them after baby was born......

Codeine is AAP aproved medicine for pregnant and breastfeeding mothers, and it is Pregnancy Risk Categegory C, witch means risk cannot be ruled out and Lactation Risk Cathegory L3, witch means it is considered moderately safe; per Hale: weakened or premature infants be observed for sedation and apnea.

I would ask now for different set of medications, because for me it appears codeine is bigger risk while taken when pregnant, Kay.

RISK CANNOT BE RULED OUT:
Adequate,well-controlled human studies are lacking, and animal studies have shown a risk to the fetus or are lacking as well. There is a chance of fetal harm if the drug is administered during pregnancy; but the potential benefits may outweigh the potential risks

Per Thomas W. Hale, PhD (Medications and Mothers' Milk, 2002, p.15):

If your dos will suggest something tell him to cross check it with Thomas W. Hale, PhD (Medications and Mothers' Milk;

Hale says:
Codeine and Morphine are generally safe in breastfeeding mothers if not overused;
Ibuprofen and paracetamol are safe to use.


General suggestions for medications in mothers' milk:



  1. assume ingestion early postnatally is minimal;
  2. assume that transfer to the infant is minimal, but always check;
  3. breastmilk with a small amount f medication in it s far superiour to formla milk;
  4. remember that even a birief interruption of breastfeeding can result in reduced milk production;
  5. calculate the Relative infant dose : if <10% then probably safe:

RID= Infant dose (mg/kg/day)
Maternal dose(mg/kg/day)
